Please be advised that I am filing a formal complaint with you concerning a violation of my right to privacy granted by the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA).
On my recent visit for diabetic care at your office, the following problems occurred.
1. Your receptionist called my name in the waiting room and asked me the reason for my visit in front of the entire room.
2. While waiting to see the doctor in an examination room, my file was placed in a pocket on the door with my name and reason for my visit prominently displayed.
3. On my way out, the receptionist stated loudly that I needed to return to see the doctor in three weeks to check if my diabetes was under control. She also read off my insurance coverage and stated the balance I owed in front of the entire waiting room.
These incidents humiliated me. I do not wish to share my personal information with anyone other than those necessary.
Please address these issues.
