Once again I am writing to you about a billing error for my hospital stay in . I have disputed this bill previously in my correspondence to you dated . A copy of this letter is enclosed.
To date you have not provided a detailed bill. Once again, I dispute the accuracy of your charges. Based on the insurance payments and explanation of benefits provided to me by my managed care insurance company, I believe that you have charged me the full rate. I should have been charged the discounted rate negotiated by my insurance company.
Please correct this error. I am still waiting for an itemized bill.
